What Is It?

DC 7804 rates scars that are painful on examination or unstable (meaning they frequently lose their covering of skin, forming open wounds or scabs). This code applies to scars anywhere on the body and is rated based on the number of qualifying scars. It is one of the most commonly claimed scar codes because many service-connected scars remain painful long after healing. A scar can qualify under this code in addition to other scar codes, allowing veterans to receive multiple scar ratings.

Rating Criteria

20%Three or four scars that are unstable or painful.
30%Five or more scars that are unstable or painful. A scar that is BOTH unstable AND painful is rated separately, with the table value increased by 10% per such scar.

For unstable scars, photographs showing the breakdown and records of treatment for open wounds demonstrate the instability. Medical records showing ongoing treatment for scar pain support the claim. Service treatment records connecting the scars to in-service injuries establish the nexus.

Frequently Asked Questions

Can I get ratings under both DC 7804 and DC 7801?

Yes. DC 7801 rates the area of deep scars, while DC 7804 rates the pain and instability of those same scars. These are separate evaluations and you are entitled to both. Make sure your claim lists both diagnostic codes.

How does the double-counting for painful AND unstable scars work?

If a single scar is both painful to touch and unstable (frequently breaking open), it counts as two scars for the purpose of DC 7804 rating. So if you have three scars that are all both painful and unstable, they count as six qualifying scars, giving you the maximum 30% rating under this code.
