What Is It?

Diffuse interstitial fibrosis (also called idiopathic pulmonary fibrosis or interstitial lung disease) is progressive scarring of lung tissue that makes it increasingly difficult to breathe. For veterans, this is particularly relevant to those exposed to burn pits, asbestos, chemical agents, or other toxic inhalants during service. The scarring is irreversible and typically worsens over time, making early documentation critical for VA claims.

Rating Criteria

10%Mild fibrotic changes on imaging with PFT values showing early restrictive pattern. FVC between 75-80% predicted or DLCO between 66-80% predicted.
30%Moderate fibrosis with FVC between 65-74% predicted or DLCO between 56-65% predicted. Noticeable dyspnea with moderate exertion.
60%Advanced fibrosis with FVC between 50-64% predicted or DLCO between 40-55% predicted. Significant exercise limitation and may require supplemental oxygen with heavy activity.
100%Severe fibrosis with FVC below 50% predicted, DLCO below 40% predicted, or requiring continuous oxygen therapy. Maximum exercise capacity severely limited.

Evidence Needed

High-resolution CT scan showing fibrotic pattern. Serial PFTs (especially FVC and DLCO) showing progression. Exposure documentation from service (burn pits, asbestos, chemicals). Pulmonologist evaluation and diagnosis. Six-minute walk test results if available.

Frequently Asked Questions

Does the PACT Act help with interstitial fibrosis claims?

Yes. The PACT Act established presumptive service connection for certain respiratory conditions related to burn pit and toxic exposures. If you served in qualifying locations during qualifying periods, you may not need to independently prove a nexus between your fibrosis and service.

My PFTs look normal but my CT shows fibrosis—can I still get rated?

Early fibrosis may not significantly affect standard PFT measures. Push for DLCO testing, which detects gas exchange problems before volume measurements decline. Even a 10% rating acknowledges the condition and allows for future increases as it progresses.

Is pulmonary fibrosis the same as interstitial lung disease?

Pulmonary fibrosis is a type of interstitial lung disease (ILD). The VA rates various forms of ILD under DC 6825 regardless of the specific subtype, focusing on the functional impairment shown on PFTs.
